How to submit your sample / Cómo enviar tu muestra

ENGLISH INSTRUCTIONS

Please follow the steps below to submit your post-vasectomy sample:

1. Collect your sample following the instructions in the video below.
2. Fill out the label on the biohazard bag.
3. Place the sample in the provided box and seal it with sturdy tape.
4. Take the sample box to a U.S. Post Office.
   • Please note the following:
     1. Postage is not prepaid. You are responsible for paying the postage.
     2. Ask for a tracking number so you can confirm when the sample arrives at our office.
     3. No need to use overnight shipping. Regular mail is sufficient. We are checking for the absence of sperm, not for live sperm.
     4. The sample should arrive within one week of collection.
5. Do NOT use UPS or FedEx. These carriers deliver by hand and may not leave packages in our mailbox if the front office is closed.
6. Your result will be available within 48 hours of receiving your sample (they will be available in the Patient Portal HERE, make sure you are registered)

INSTRUCCIONES EN ESPAÑOL

Sigue los pasos a continuación para enviar su muestra post-vasectomía:

1. Recoge tu muestra siguiendo las instrucciones del video a continuación.
2. Completa la etiqueta de la bolsa de riesgo biológico.
3. Coloca la muestra en la caja provista y ciérrala con cinta adhesiva resistente.
4. Lleva la caja de la muestra a una oficina de correos (USPS).
   • Ten en cuenta lo siguiente:
     1. El costo de envío no está prepagado. Serás responsable de pagar el envío en la oficina postal.
     2. Solicita un número de seguimiento para confirmar cuándo llega la muestra a nuestra oficina.
     3. No es necesario utilizar el envío urgente. El correo postal regular es suficiente. Recuerda que estamos verificando la ausencia de espermatozoides, no si se encuentran vivos.
     4. La muestra debería llegar en un plazo de una semana desde su recolección.
5. NO utilices UPS ni FedEx. Estas compañías entregan personalmente sus paquetes y es posible que no dejen paquetes en nuestro buzón si la oficina principal está cerrada.
6. Tus resultados estarán disponibles dentro de 48 horas luego de recibir tu muestra (se enviarán al Patient Portal AQUÍ, asegúrate de registrarte)
